Mechanism Detail
DBS delivers continuous electrical pulses to the subthalamic nucleus (STN) or globus pallidus interna (GPi) in Parkinson's, modulating basal ganglia circuits. The mechanism involves both local inhibition and network-level effects: regularizing pathological oscillatory activity, releasing thalamocortical relay neurons from excessive inhibition, and modulating neurotransmitter release.
Clinical Status
FDA-approved for Parkinson's, essential tremor, dystonia, and OCD. Adaptive DBS (responsive stimulation) in development. Trials exploring DBS of fornix/nucleus basalis for Alzheimer's showed mixed results.
