Lecanemab (Leqembi)

An anti-amyloid-beta monoclonal antibody approved for early Alzheimer's disease. Targets soluble protofibrils to reduce amyloid plaque accumulation.
Mechanism Detail
Lecanemab selectively binds large soluble amyloid-beta protofibrils, which are considered the most neurotoxic species. It promotes their clearance via microglial phagocytosis, reducing both soluble and deposited amyloid. ARIA (amyloid-related imaging abnormalities) is the primary safety concern.
Clinical Status
FDA full approval (2024). CLARITY AD trial showed 27% slowing of cognitive decline over 18 months. Post-marketing studies ongoing.
